VFIS Claims Reporting

How to File a Property and Casualty ClaimWe believe that the local agent is, and should remain, an integral part of the insurance process. A vital first step in the successful handling of a claim is prompt and accurate notification of information to the local agent.
When reporting a claim, you will be asked to provide basic information, such as:
- Date, time, and location of the occurrence.
- General description of what happened.
- Identity and contact information for all persons involved.
- Description of the damages and/or injuries.
- Description of the automobiles, property, persons and witnesses involved, including addresses and telephone numbers.
- If the claim involves an automobile accident or a crime, the name of the police department and the report number.
- Provide assistance to injured persons.
- Protect your property from further damage.
- Do not divulge information to anyone other than your assigned claim handler or their authorized representative.
- In the event a suit is filed against you or your organization, contact your local agent immediately and provide copies of the suit papers.
How to File an Accident and Sickness Claim
When reporting a claim to the local agent, you will be asked to provide basic information, such as:
1.) A VFIS Accident/Sickness claim report form:
a.)The top section must be completed in its entirety by the injured person and signed. Please provide a clear description of the activity and circumstances surrounding the injury/illness.
b.) The bottom section must be completed by a fire company official (other than the injured person), certifying the information on the report is true.
For incidents involving a fatality:
1.) VFIS Accident/Sickness claim report form should be completed by the next of kin.
2.) Additional information should be provided as it becomes available, including:
- VFIS Beneficiary card or a letter from an official of the insured Organization stating there is no beneficiary card on file.
- Certified copy of the Death Certificate.
- If available, an autopsy report and/or hospital records.
- If available, the police report and newspaper articles.
